and are happy to file your claim. You should contact your insurance
company to determine the coverage available to you and for information
regarding your deductible and co-pay.

Minors
Children under the age of 18 Must be Accompanied by their parent or legal
Guardian. You must have proof of Power of Attorney from the court. DSS must
be present if they have custody of a child and the child is in foster care.
Legal guardians must have papers from the court showing legal guardianship.
CHILDREN SHOULD HAVE AN AGE AND WEIGHT APPROPRIATE CAR SEAT.
Cancellation Policy
If you are calling to cancel your procedure please call your doctor’s
office and let them know you are canceling your procedure.
Pre-Procedure Instructions
- DO NOT EAT OR DRINK ANYTHING, including water, candy, gum, and
tobacco products.
- You should bathe or shower with soap and water before you arrive for
your procedure.
- Upon arrival at Cleveland Ambulatory Services, you will be asked to:
- Empty your bladder.
- Completely undress and put on a gown.
- Remove dentures, contacts, glasses, hair clips, jewelry, etc.
- You may be given some medication by injection to make you sleepy.
- DO have a responsible adult to accompany you. This person should
remain at Cleveland Ambulatory Services until you are ready to go home
and should stay with you overnight.
